CASE MANAGEMENT
Develops case plans with clients for the evaluation, treatment, and care of individuals who are unable to arrange services for themselves due to age, illness, disability, or other difficulties. Case managers coordinate the delivery of the services, ensure that services are obtained, and monitor clients' progress. Service is designed to be a short term service.
